We're not the right fit for
- Hospice

- We can't admit residents with aggressive or behavioral-health needs.
- Skilled nursing tasks
- No nurse on site, so no catheter changes, wound vacs, IVs or injections other than insulin.

How a referral works
Send the profile
Call or use the form: diagnosis, ADLs, mobility/transfers, meds (insulin?), behaviors, payer and timeline.
We confirm fit
Usually the same day. If it's not a fit, we'll say so plainly.
Tour or assessment
Family tours; we complete a care assessment in person or at the hospital/rehab.
Paperwork & move-in
Physician's order, med list, TB screening and ALTCS authorization if applicable. Then move-in.

Do you pay placement agent fees?
Yes. We work with placement agents and pay their fees. Call us to set up an agreement.
Do you take ALTCS?
Yes, all three Maricopa County ALTCS plans: Mercy Care, UnitedHealthcare Community Plan and Banner – University Family Care.
Do you accept wandering / exit-seeking residents?
Yes. We're licensed for directed care and our building and backyard are secured.
Can you take a Hoyer lift resident?
Yes.
Can you manage insulin?
Yes. Caregivers administer insulin per physician orders.
Can you take a resident with a catheter?
Yes, we provide catheter care. Catheter changes must be done by home health because we don't have a nurse on site.
Do you take hospice or behavioral-health residents?
No. We don't accept hospice or behavioral-health residents at this time.
Are rooms private or shared?
We have 13 companionship (shared) rooms and 1 private room. Roommates are matched carefully, so include gender and personality notes with a referral.
